If you’re not currently appointed with Delta Dental of North Carolina and would like to represent us, please send us the following forms and we’ll submit your paperwork to the North Carolina Department of Insurance – we’ll also pay your appointment fees:

Application for Limited Representative License
Taxpayer Identification Number Request (IRS form W-9)
Producer Agreement/HIPAA Agreement form
Copy of your current LA&H or A&H license

For new and renewal sales of our Delta Dental Premier^® program and Indemnity program (launched in June 2008) you’ll receive 10% flat commission.

Direct deposit is available and we pay all annual appointment fees.
